I’m assuming here that the figure is not supposed to be a clown but a transsexual (specifically a M to F one).
That is the only way I can think of how the cartoon could possibly be interpreted as meaning anything at all, specifically “Transsexuals look like clowns, LOL.”

Yep. I initially went “huh?” but then saw that the “clown”, the mom & kid were riding on the subway/a bus and the “clown” was not doing clown-stuff…

…Joke’s on the cartoonist; Here’s some photos of actual transpeople (All the men were born ‘female’ and the women were born ‘male’).
Non of them look like Bozo, Ronald or the monstrous Pennywise.
And again; IS EVERYBODY IN THAT COMIC SUPPOSED TO BE A DUCK?
For me, this caused my possible movie idea about road trippers discovering they slid into a parallel world somewhere on the road to resurface.
Here, they would now be IN A WORLD where loads of bigoted ideas from our world are actually true:
- All trans people really do look like cross-dressers who don’t ‘pass’ at all.
- Gay men are all femme while gay women are all butch. And gays really are more prone to sexually abusing children than straights.
- Bisexuals really are ‘just on a stopover to Gayville’.
- The incel view of women is spot on.
